Documentation Guidance

Accommodation Documentation

Students should provide documentation that is readily available and speaks to the nature of their condition/disability.  If additional information is needed to support an accommodation request, students may be asked to provide updated and/or relevant documentation. All accommodations are determined on a case-by-case basis.

Examples of Documentation

Many forms of documentation can support an accommodation request including but not limited to:

  • Letter from a provider (therapist, doctor, etc.)
  • Neuropsychological or psychological evaluations
  • Prescription/medication aligned with condition (picture)
  • Medical chart or MyChart stating diagnosis (picture or scan)
  • IEP, 504, or evaluation reports from high school
  • SAS Documentation Form (pdf) (completed by qualified professional)
  • Disability or Veteran benefits qualification letter

Documentation Considerations

Documentation is used to assist in the determination of accommodations and facilitate access. Helpful information that could be shared in your documentation includes, but is not limited to:

  • Statement of medical condition/diagnosis
  • Severity and impact of the condition
  • Expected progression or stability of condition
  • Description of functional limitations and/or barriers
  • Description of current and past accommodations, services, and/or medication(s)

Housing Guidance

Housing accommodation requests require more thorough information documenting how conditions lead to disability, specifically in residential living settings. Using the preferred supporting documentation (outlined below)  

For Room and Dining accommodation requests, the Accommodation Documentation Form (pdf) is the preferred supporting document. This should be completed by the provider that can best share information about how diagnosed condition(s) could/do impact you in living situations and how the requested accommodation will support access.  

For Support Animal (also referred to as ESA) requests, the Support Animal Documentation Form (pdf) is the preferred supporting document. This should be completed by the provider that can best share information about how diagnosed condition(s) are disabling and the benefit an animal can bring to the quality of one's experience on campus.  

At times, other documentation can support housing requests. A detailed letter from a qualified professional, who can share information about the student's condition/disability and related housing accommodation needs, may provide enough support to determine. We encourage students to contact us with any questions about documentation that can support their housing requests.

For timely review, please attach relevant disability-related documentation to the Accommodation Request Form at time of submission.

All documentation (and its contents) provided to SAS is confidential and will not be shared or distributed outside of the department without students consent. Students requesting SAS release confidential information must complete the Consent to Release Form (pdf).
